Request a Transcript

To request an official or unofficial Taunton High School transcript, complete the Transcript Request Form and submit it to the THS Registrar using one of the available submission methods.

What You'll Need:

Before completing the form, have the following information available:

Name at the Time of Graduation
Your name as it appeared while attending Taunton High School.

Graduation Year
The year you graduated.

Date of Birth
Required to help verify your records.

Recipient Information
If your transcript is being sent directly to a college, employer, or scholarship organization.

Complete the Form

When completing the Transcript Request Form, you will be asked to:

Indicate whether you are requesting an official or unofficial transcript.
Select the purpose of your request (college, scholarship, employment, or other).
Choose how you would like your transcript delivered (email, mail, or pick up).
Provide the name and address or email address of the recipient, if applicable.
